The firm’s founder, Michele R. Maddox, CPA is a native of Birmingham, Alabama and a 1982 graduate of the University of Alabama at Birmingham. She holds a Bachelor of Science degree in accounting. Her public accounting career has included bookkeeping, payroll, and tax services for businesses and individuals, and audits of governmental and nonprofit entities.
Michele R. Maddox, CPA is a native of Birmingham, Alabama and a 1982 graduate of the University of Alabama at Birmingham. She holds a Bachelor of Science degree in accounting. Her public accounting career has included bookkeeping, payroll, and tax services for businesses and individuals, and audits of governmental and nonprofit entities.
Michele is the founding partner of Maddox & Associates located in Birmingham, Alabama. The firm specializes in tax planning and strategy to minimize tax burden. Home based business is a prime area for tax confusion, and Michele has been dedicated for more than 20 years to research and education in the field for the home-based business owner.
Michele also specializes in the technology area. She has been instrumental in the success of the computer consulting divisions of three large accounting firms in the Southeast. Michele has over 28 years experience with network administration and computer software consulting, specializing in restaurants, law firms, medical offices, service businesses, and direct sales consultants. Michele has even written specialized software for dance studios, event management companies, and service related businesses. Michele is a current Quickbooks Proadvisor, working with Quickbooks software for over 25 years.

In addition to the Federal Income Tax Withholding Form, W-4, your employee will also need to fill out an Alabama State Income Tax Withholding Form known as the "A4 Form." The A4 form essentially ask the same questions as the W-4, but for this form is for the State of Alabama. Remember that if the employee does not fill out a A4 then taxes must be withheld at the highest level possible.
Form I-9 is used for verifying the identity and employment authorization of individuals hired for employment in the United States. All U.S. employers must ensure proper completion of Form I-9 for each individual they hire for employment in the United States. This includes citizens and noncitizens. Both employees and employers (or authorized representatives of the employer) must complete the form. On the form, an employee must attest to his or her employment authorization. The employee must also present his or her employer with acceptable documents evidencing identity and employment authorization. The employer must examine the employment eligibility and identity document(s) an employee presents to determine whether the document(s) reasonably appear to be genuine and to relate to the employee and record the document information on the Form I-9. The list of acceptable documents can be found on the last page of the form. Employers must retain Form I-9 for a designated period and make it available for inspection by authorized government officers.
Do not file Form I-9 with USCIS or U.S. Immigrations and Customs Enforcement (ICE). Employers must have a completed Form I-9 on file for each person on their payroll who is required to complete the form. Form I-9 must be retained and stored by the employer either for three years after the date of hire or for one year after employment is terminated, whichever is later. The form must be available for inspection by authorized U.S. Government officials from the Department of Homeland Security, Department of Labor, or Department of Justice.
If you are hiring a new employee, they will need to fill out a W-4, A4 and I-9 at the time of hiring. If an existing employee would like to change their withholdings, they would need to fill out the W-4 as well as the A4 Forms.
Employees may present one selection from List A or a combination of one selection from List B and one selection from List C as identification.
